Welcome to the New York Independent Assessor Program's Online Scheduling Assistant

The application can be used by Medicaid members or their authorized Representatives to schedule an annual assessment.

To use the Online Scheduling Assistant for the first time

  1. Register. The registration will ask your name, date of birth, New York State Benefit Identification Number, Social security number, and email address. You will be asked to create a password. To register, click here

  2. SignIn. After you've registered, you can sign into the Online Scheduling Assistance at any time to schedule your annual reassessment, when you sign in, you will be taken to your "My Dashboard" page, where you can begin the scheduling process. To sign in, click here
